Prior Authorization

BCBSKC may require prior authorization before certain medical services and/or medications are performed or dispensed. Click one of the three categories below to browse prior authorization forms.

Please allow at least two (2) business days from the date of receipt of all necessary information for a determination on a prior authorization request. To check the status of a prior authorization, call the customer service number listed on the Member Identification Card.

Incomplete prior authorization requests and forms may result in a denial. All member information is strictly confidential. Contact BCBSKC Customer Service to verify member eligibility and benefits.
